4C Provider Insurance Program

Insurance Plans | Qualifications to Enroll | Contact Information

In an effort to recruit and retain quality child care providers, 4C is pleased to present to Family Child Care Providers and to Child Care Center staff various personnel benefit plans:

To qualify to enroll in the 4C Provider Insurance Program, you must meet one of the following standards:

Family Child Care Providers

  • Family Child Care Providers in Kentucky must be a Certified family child care home, OR a Type II licensed child care home, in good standing in the state of Kentucky
  • Family Child Care Providers in Ohio must be a 4C registered child care provider OR a CACFP family child care home in Hamilton, Clermont, or Butler county, OR a Type A Provider, in good standing in the state of Ohio
Child Care Centers
  • Child Care Centers in Kentucky must be a Type I licensed child care center, in good standing in the state of Kentucky
  • Child Care Centers in Ohio must be a 4C registered child care provider, OR a Type A provider, OR a licensed child care center, in good standing in the state of Ohio

All of the plans in the program are available to family child care providers and to staff of child care centers. You can choose to enroll in one, a combination of any, or all of the plans offered in the program—it's your choice. Some plans require a minimum number of staff in child care centers to enroll—but in most cases, we could treat you the same as a family child care provider.

To enroll in the any of the plans in the program, you will need to:

  • complete the appropriate enrollment forms,
  • complete the sponsorship agreement,
  • make payment on the first month's premium and administrative fee at the time of enrollment. We can accept payment by check, money order, cash, credit card or electronic debit.

The administrative fee is a monthly payment to be paid with each month's premium. The administrative fee is the only fee 4C receives from the Provider Insurance Program. It helps offset our costs of mailing processing applications, and making payments to the insurance companies.

4C is NOT an insurance broker or representative of the insurance companies for any of these plans. 4C does NOT receive any commission on any of these plans. 4C is NOT responsible for any issues a participant may have with the insurance company other than payment to the insurance companies.

Plan Options in the 4C Provider Insurance Program

Medical Insurance Plan

4C provides child care professionals access to medical insurance through a company called Benefits Protect. The number to call is 1.866.210.2999 to connect with a counselor who will gather information to help determine the best medical insurance situation for each person.

The first option will be major medical insurance through insurance companies such as Humana or Aetna. But if that is not available due to medical conditions or affordability, you can choose from options with a limited medical plan. If that option does not work for whatever reason, you can still receive a discount medical savings card.

Please note two very significant differences with the medical plan as opposed to dental and AFLAC plans:

  • 1) Providers will work with Benefits Protect to determine best insurance plan options and not 4C. 4C does not have any role to play in this medical insurance plan other than to provide providers with the number to call.

  • 2) Because 4C will not have a role to play with the medical insurance plan, providers will provide Benefits Prtotect with the information needed to make payments to them each month and all claim questions for Benefits Protect should be directed to their 800 number or the insurance company with which you enroll and not 4C as we will have no relationship with these insurance companies.

Dental Insurance

Assurant/Fortis Dental Care

Dental insurance: with a choice of a managed care option where you must pick a dentist from the company directory with lower premiums and better coverage vs. the option of choosing your own dentist where benefits are limited to an agreed dollar amount. Discount plan on vision expenses included with this plan. (Managed care plan NOT available in Indiana.)

Disability Insurance

AFLAC (available only in Greater Cinti/NKY area)

Short-term disability insurance to cover lost income for provider while off of the job for up to 90 days offered through AFLAC. Monthly premiums vary depending on income levels and age. For example, if you need $700 per month coverage and are between 18 and 49 years of age, the monthly premium would be $18.20; between 50 and 65 years of age, the monthly premium would be $20.30. These rates vary with income. $1.00 per month administrative fee is charged.

Accident Expense Insurance

AFLAC (available only in Greater Cinti/NKY area)

Personal Accident Expense insurance through AFLAC covering ER, hospital stays, and lump sum injury benefits for on or off the job injuries.

Prepaid Legal

Prepaid Legal Services—a plan where you can pay on a monthly basis for future legal costs for items such as consultation with a lawyer, will preparation, legal document review, home-based business legal aid, IRS audit legal services and much more. Monthly premiums range from $15 to $25/month depending on chosen coverage.

Liability Insurance Programs

Acordia

Liability Insurance Program for child care centers. A broker from Acordia/Rauh will work with you in obtaining quotes for the best coverage for the facility.


4C is the administrator of the dental and AFLAC plans- meaning you pay 4C for the monthly premiums and we send the payments to the individual insurance companies. 4C has arranged with each of these insurance companies to obtain these rates for child care providers.

What can you do to enroll in one or more of the plans or to obtain more information?

For medical, call 1.866.210.2999. For all other plans, call our Provider Insurance Line at 513.221.0033, ext. 1355 to ask us to mail you more information. We can mail enrollment forms for you to complete for the Dental insurance plans. Leave your name, address, daytime phone number, and what plans you are requesting information. Please be specific about the plan for which you want information.
