The Provider Portal will not be available on Saturday March 27 or Sunday March 28. However, normal operating hours, 7 am to 7:30 pm, will resume on Monday, March 29, 2010.

Once you receive a letter from the State with your personal information, contact 4C and, at no charge, we can help you:
- access a computer and the Internet.
- obtain a free e-mail account.
- register and complete your agreement online.
- establish your rates.
- and anything else you need to get ready and get registered!
For a limited time—in March only!
Access computers with online capability at the 4C Cincinnati location Monday through Friday, 9 a.m.–5 p.m., and Saturdays from 9 a.m.–noon.
If you need assistance, call to set up an appointment during one of these times in March at the 4C Cincinnati location:
Tuesdays: Noon – 2 p.m.
Thursdays: 5 p.m. – 7 p.m.
Saturdays: 10 a.m. – noon
Helpful Hints
1. Do not use the back button or the refresh button on your browser when using the portal. You will have to log back into the portal.
2. If you wish to print your forms, this must be done after you submit them to ODJFS. After you submit the forms for the first time, go back into the form, and select the PRINT button at the bottom of the form. After you print the form, and you have already successfully submitted the form, you can select cancel. No new information will be sent to ODJFS. If you make changes to the form after you successfully submit it the first time, then you must choose the check mark box at the bottom of the form and submit again.
3. If you get logged out due to a security violation, you are locked out for 36 hours. Do NOT try to log back in again for 36 hours. The 36 hour clock starts at the last time you log in.
